Certified Medication Technician needed for all aspects of medication administration within the scope of his/her certification and under the supervision and direction of a Registered Nurse. Caregiver will perform medication administration according to the scope indicated on the Care Plan developed by the Registered Nurse. The CMT will report directly to the agency Supervising Nurse.
Essential Functions Includes:
- Carries out duties indicated on Care Plan
- Vital signs on every visit
- Observe clients and their environments and reports behavior, physical and/or cognitive changes and/or changes in living arrangements while caring for the client in the home to supervisor and records findings on the appropriate visit note
- Report suspected abuse & neglect immediately (within one hour)
- Complete, maintain records of, and submit documentation of client’s care in accordance with agency policies and procedures
- Organize work to meet client’s needs
- Listen actively to client and family members when expressing feelings and concerns
- Reinforces nurse instructions to client and family
- Assistance with self-administration of medications or administration of medications or other remedies when order by a physician, assists with medication reminders
- Safely prepares, administers, and charts oral, topical, and suppository drugs. The medication aide who prepares the medicine administers same.
- Checks and prepares each dose immediately preceding administration.
- Follows administration protocol:
- Checks medication with transcribed or original order.
- Checks all information on label or package for accuracy and clarity.
- Clearly identifies the client with proper medication. * Considers the following when administering a medication.
- General use of the medication
- Therapeutic action
- Usual dosage
- Factors modifying dosage and effects
- Precautions and contraindications
- Antidote, if known
- Route and frequency of administration
- Signs of deterioration of the medication * Checks with the supervising nurse if there are any questions regarding any medication.
- Records administration of the medication on client’s medication administration record. Discards medication refused or otherwise not given and records on medication administration record.
- Reports medication errors or reactions to medications immediately to the supervising nurse, who, in turn, reports to the client’s attending physician and completes the necessary documentation.
- Performs accurate pulse and blood pressure and recognizes their significance in drug therapy.
- Signs and has access to the controlled schedule drug cabinet.
- Administers medication only upon a written physician’s order.
- Follow agency policy when disposing of biohazardous materials, labeling, and handling of all materials.
- Participates in multi-disciplinary meetings and QA activities.
- Attend orientation, in-service training sessions and staff meetings
- Contact appropriate administrative staff during evening and night hours as required
- Using communication devices: Including computers, tablets, or telephones
- Performs other duties as assigned by the supervisor, and/or administrator
Qualifications/Requirements:
- CMT certification in the State of Maryland
- Current CPR -BLS for healthcare providers
- Current Immunization records (TB/PPD, MMR, Varicella, HBV)
- Successful completion of a skills competency evaluation & exam
- Able to withstand prolonged standing and walking with the ability to move or lift at least 50 pounds.
- Working knowledge of sanitation, personal hygiene and basic health and safety precautions applicable to work at the agency.
- Working knowledge of infection control procedures and safety precautions
- Ability to understand English and follow oral and written instructions
- Ability to document and communicate pertinent information
- Ability to establish and maintain effective working relationships with patients and agency staff
- At least one year medication administration experience in a homecare or healthcare setting required
- Great Communication skills and able to work independently
- Valid driver’s license and insured personal vehicle
- Cleaning driving record

- Solid computer skills, prior experience with electronic medical records (EMR) preferred
