Personal Care Provider (PCP) provide services to individuals in their homes who need assistance caring for themselves as a result of old age, sickness, disability, and/or other inflictions. Home care personal care providers are responsible for ensuring that services including Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) are delivered in a caring and respectful manner in accordance with agency policies and industry standards. Personal Care Provider will report directly to Supervising Nurse.
Essential Functions Include:
- Assisting with or completing Activities of Daily Living (provide grooming, dressing, and bathing guidance if needed)
- Assisting with or completing Instrumental Activities of Daily Living
- Assistance with tasks requiring judgment to protect from harm or neglect
- Provide companionship
- Provide light housekeeping of the client’s immediate areas and home management
- Assists with or complete meal preparation and planning
- Assists client with ambulation and mobility around the house and outside (doctors appointments, walks, etc)
- Perform client’s shopping or accompany client when they shop or run errands if needed
- Assist with using telephone or other communication means
- Assist with planning and decision making
- Assists with self administration of medications and medication reminders
- Relay any changes in client’s physical condition, behavior, or appearance to the office
- Assist with or provide laundry and lined service to include washing clothes and changing bed linen
- Assist client with organizing and managing home affairs
- Provide assistance and planning of client’s recreational activities and events
- Submit documentation of client’s care in accordance with agency policies and procedures
- Maintain safe environment for client
Qualifications/Requirements:
- High school diploma or G.E.D
- Current CPR -BLS for healthcare providers
- Current immunizations: (TB/PPD, MMR, Varicella, HBV)
- At least 1 year experience as a homemaker/companion in homes or facilities (required)
- Successful completion of a skills competency evaluation & exam
- Valid driver’s license and insured personal vehicle
- Clean driving record
- Reliable transportation
- Outstanding communication, time management, and interpersonal skills

- HIPAA compliance
- Solid computer skills, prior experience with electronic medical records (EMR) preferred
- Ability to travel to cases as assigned
- Must be able to pass both a criminal background check and drug screening
Job Type: Per-Diem
License: Valid Driver’s License (preferred)
Schedule: Available Monday-Friday, Weekends
Experience: Homecare: 1 year (preferred)
Hours per Week: 12-30 hrs/wk
