Under supervision of a registered nurse the Licensed Practical Nurse is responsible for providing quality care to clients and performs technical skilled care in patient’s home in compliance with nursing standards, federal, state, local government regulations and company clinical policies. Service will be provided to adult and geriatric clients on a per visit basis in Baltimore City, Baltimore County, and all surrounding Counties. LPN reports directly to the Supervising Nurse.

Essential Functions Include: (but are not limited to)

  • Administration and Documentation of Medications
  • Performing wound care, treatments, and procedures
  • Provide direct and individualized nursing care to assigned clients based on nursing standards and under the supervision of the Registered Nurse.
  • Ensures quality and safe delivery of home health services to clients and families/caregivers
  • Implement plan of care formulated by physician and Registered Nurse.
  • Provides accurate and timely documentation consistent with plan of care.
  • Completes a progress note for each visit, and submit progress notes to the agency daily.
  • Observe, records, and reports to supervising nurse the client’s response to treatment and changes in the client’s condition.
  • Assess and provides client and family caregiver education and information pertinent to diagnosis, medications, nutrition, and plan of care.
  • Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.
  • Participates in coordination of home health services with Registered Nurse and clinical management.
  • Assists clients in learning appropriate self-care activities.
  • Uses equipment and supplies effectively and efficiently.
  • Documents and reports activity as required.

Qualifications/Requirements:

  • Current LPN licensure in state of Maryland
  • Graduated from an accredited and approved nursing program
  • Valid driver’s license and insured personal vehicle
  • Clean driving record
  • Reliable transportation
  • Current CPR-BLS for healthcare providers
  • Current immunizations records (TB/PPD, MMR, Varicella, HBV)
  • Minimum of two (2) years of clinical experience with at least one (1) year of experience working in home health or related healthcare organization
  • Demonstrated ability to supervise and direct staff
  • Possess exceptional nursing assessment skills
  • Proven ability to build quality and implement improvements within the operation
  • Excellent communication, time management, and interpersonal skills
  • Ability to work independently
  • HIPPA compliance
  • Solid computer skills, prior experience with electronic medical records (EMR) preferred
  • Ability to travel to cases as assigned
  • Must be able to pass a criminal background check and drug screening
