Touching Lives With Hands LLC is seeking an experienced Registered Nurse to join our team. The Registered Nurse will be responsible for supervision of LPN’s & caregiver staff along with employing the nursing process to develop and implement plans of care for patients requiring home health care services. In addition to specific and skilled treatments and procedures, the Registered Nurse provides health teaching to patients and caregivers. The Registered Nurse ensures adequate service provision, including scheduling of visits based on patient’s identified needs and priorities for care. Complies with all Homecare policies and procedures. Service will be provided to adult and geriatric clients on a per visit basis in Baltimore City, Baltimore County, and all surrounding Counties. Registered Nurse reports directly to the Director of Nursing & Managing Director.

Essential Functions Includes:

  • Will complete the initial assessment/evaluation visit of the clients health, function, and psychosocial condition for each new client who requires skilled services and assistance with ADL’s
  • Regularly re-evaluates the client’s care needs
  • Initiates or reviews the plan of care, make necessary revisions and assign appropriate personnel
  • Provides or delegates those services requiring substantial and specialized nursing skill
  • Initiates appropriate preventive and restorative nursing procedures
  • Prepares clinical, progress notes and summaries of care
  • Counsels the client and family in meeting nursing and related needs
  • Assists in the development of organizational clinical goals
  • Assesses competency of clinical staff and ensures staff is assigned in a manner that promotes quality, continuity, and safety of a client’s care
  • Plans and implements in–service and training programs for individuals who will provide client care
  • Conducts performance evaluations of caregiving staff
  • Provide periodic on-site supervision of care at least every forty-five (45) days or greater frequency to clients based on medical or clinical status

Qualifications/Requirements:

  • Registered Nurse with current license to practice in the State of Maryland
  • Degree from an accredited Registered Nursing program
  • Minimum two (2) years of clinical experience with at least one (1) year of experience working in home health or related healthcare organization
  • Demonstrated ability to supervise and direct staff
  • Possess exceptional nursing assessment skills
  • Proven ability to build quality and implement improvements within the operation
  • Excellent communication, time management, and interpersonal skills
  • Excellent customer service skills and ability to deal tactfully with clients and staff
  • Current CPR -BLS for healthcare providers
  • Current Immunization records (TB/PPD, MMR, Varicella, HBV)
  • Valid driver’s license and insured personal vehicle
  • Clean driving record
  • Reliable transportation
  • HIPPA compliance
  • Solid computer skills, prior experience with electronic medical records (EMR) preferred
  • Ability to travel to cases as assigned
  • Must be able to pass both a criminal background check and drug screening
